Hello therapist/service coordinator,
please follow the instructions in order below to apply for services for your client.
(1) Click the “Service Coordinator Info” button below to enter YOUR information.
(2) Click the “ROI Download” button below to download our Release of Information form. This form must be signed by the client’s parent/caregiver and faxed back to us at 317-271-7459.
(3) Click the “Apply” button below to fill out our Service Application. Please indicate that you are filling out this application for “Someone else.” Under “Your info,” enter the PARENT/CAREGIVER’s information. Then fill out the rest of the application accordingly.
After you have completed steps 1-3, we will contact you with next steps to help you and your client complete additional paperwork and schedule services with us.
